To complete forms using Microsoft Word:

This option allows users who have Microsoft Word to complete the application and email as an attachment. Once the application is complete, go to the menu bar at the top and select File, Save As, and name the document with an appropriate file name. Go into your email program and attach the file you just saved and send the completed form to INVISION at:

- Each field to enter data has a limited number of characters.
- Please use abbreviations when possible (Ex: Ave. not Avenue)
- To move from field to field, use the "Tab" key on your keyboard.
- To enter a "check" mark, simply mouse click within the desired box.
